Inter-Reader Agreement of ATA Sonographic Risk in Thyroid Nodules with Bethesda Category III Indeterminate Cytology

چکیده

Background: Substantial inter-observer variation has been documented in the recognition and description of specific sonographic features as well for ATA risk (ASR). This raises question if stratification proposed by guidelines is reproducible applicable nodules with indeterminate cytology. The aim study was to determine inter-reader agreement (IRR) among radiologists using 2015 ASR thyroid nodules. Methods: Three board certified who were blinded clinical data each other, interpreted ultrasound findings 179 that had Bethesda III classified into high suspicion (HS), intermediate (IS), low (LS), very (VLS). Echogenicity, composition, shape taller than wide, vascularity, type margins, presence calcifications also described. Results: majority consensus revealed 28%, 27%, 39% 5% described high, intermediate, ASR, respectively. near perfect (k 0.82 CI 95% (0.77–0.87)). Nodules paired a higher (HS + IS) lower (LS VLS) categories substantial 0.7) both categories. Conclusion: A readers observed when stratifying cytology ASR.

Thyroid Nodules with Indeterminate Cytology

To the Editor: Alexander et al. (Aug. 23 issue)1 report findings from a prospective, multicenter gene-expression classifier validation study that raises several questions. For the 265 indeterminate fine-needle aspirates they tested, the geneexpression classifier had a sensitivity of 92%, a specificity of 52%, a positive predictive value of 47%, and a negative predictive value of 93%. Why the ob...

Role of sonographic diagnosis in managing Bethesda class III nodules.

BACKGROUND AND PURPOSE Bethesda class III cytology is an important limitation of the US-FNA in assessing thyroid nodules. This study aimed to assess the diagnostic efficacy of US in evaluating thyroid nodules with Bethesda class III cytology. MATERIALS AND METHODS From January 2008 to December 2009, 1036 patients with 1289 thyroid nodules diagnosed by US and subsequent US-FNA biopsy were enro...

Preoperative diagnosis of benign thyroid nodules with indeterminate cytology.

BACKGROUND Approximately 15 to 30% of thyroid nodules evaluated by means of fine-needle aspiration are not clearly benign or malignant. Patients with cytologically indeterminate nodules are often referred for diagnostic surgery, though most of these nodules prove to be benign. A novel diagnostic test that measures the expression of 167 genes has shown promise in improving preoperative risk asse...

Risk stratification of thyroid nodules with Bethesda category III results on fine-needle aspiration cytology: The additional value of acoustic radiation force impulse elastography

To assess the value of conventional ultrasound, conventional strain elastography (CSE) and acoustic radiation force impulse (ARFI) elastography in differentiating likelihood of malignancy for Bethesda category III thyroid nodules. 103 thyroid nodules with Bethesda category III results on fine-needle aspiration cytology (FNAC) in 103 patients were included and all were pathologically confirmed a...

Repeat Diagnoses of Bethesda Category III Thyroid Nodules: What To Do Next?

OBJECTIVES To assess the malignancy rates of thyroid nodules repeatedly classified as Bethesda category III on fine needle aspiration (FNA), and to suggest management guidelines for these lesions. METHODS This is a retrospective study that included 395 thyroid nodules categorized as Bethesda III undergone either surgery or ultrasound (US) follow-up. There were 67 nodules classified a second t...
